Understanding the Urethral bulking Treatment

Stress incontinence is caused by a weak urinary sphincter (a ring of muscles that regulates the flow of urine out of your bladder) or weak support to it from muscles of the pelvic floor. And so, whenever the pressure climbs as in coughing, sneezing or any type of physical exertion, your urine leaks out from your bladder. 

Thus, if you experience an incontinence during some stressful activities, you definitely need our urethral bulking treatment to provide you with a much-needed breather. 

Urethral bulking is a surgical procedure aimed at strengthening the wall of your urinary tract to withstand a build-up of pressure during activities such as sneezing etc. 

The procedure involves “bulking” the wall of the urethra with a man-made material. The material gets injected below the urethral lining or at the bladder outlet and helps these structures stay closed against an elevated pressureThe result is a strong tract that can effectively manage pressure surges and avoid incontinence.

Preparations for Urethral Bulking

You’ll probably face the following prerequisites for your incontinence procedure:

EPAQ Questionnaire

This questionnaire is directed to assess functions and symptoms related to your bladder, vagina and general bowel habits. The questionnaire is clocked in twice, once before the surgery and then 6 months after your Incontinence procedure. It’s very simple and can be done both at home and in a clinical setting.

Urodynamics

These tests are done to check the functional dynamics of your bladder, to see how well it reacts to filling pressures. The tests involve inflating the bladder with water (injected through a thin tube) and record how quick it empties. The tube is removed as soon as the test is over. It is an uncomplicated procedure that can be easily done in a clinical setting.

Pre-Op

You’ll likely be called up for pre-operative examination to get an idea about your general health status. The examination is usually carried out by a nurse. More so, we also advise you on medicines you should take for pain relief on return to your home, all of these taking into account your status of health.

Procedure For Urethral Bulking

Urethral bulking is not a very complicated procedure. It can be done under local anaesthesia or general anaesthesia. However, we prefer carrying it out under local anaesthesia in an out-patient setting.

The procedure begins with passing a small camera along the urethra to visualise it. A bulking material (man-made) is then inserted into the urethral wall at multiple sites via a fine needle. The added material provides the wall with the necessary strength to withstand pressure-buildups and avoid incontinence. 

Once you have had your urethral bulking procedure, you’ll be asked to pass urine. This is done to see if you face any difficulty in passing urine (owing to a thick urethral wall). If so, a catheter will be inserted to help you empty your bladder. Also, you may have to carry it (the catheter) home. The patient is kept on a catheter, usually for a few days, after which it is removed.

What is Urethral Bulking?

Urethral bulking is a preferred treatment option for stress incontinence. It involves injecting a bulking material into the wall of the urethra or bladder. The material provides the necessary strength to withstand and increased pressure and avoid stress incontinence.

What happens during the procedure?

The procedure is carried out under local or general anaesthesia. It begins with passing a small camera into the Urethra to visualize it. A bulking material is then inserted into its wall via a thin needle at multiple points. It is generally an uncomplicated procedure and can be early carried out in a clinical setting.

What do I need to do before my Urethral Bulking procedure?

You'll be asked to fill a questionnaire to assess your bladder vagina and general bowel symptoms. The questionnaire can be easily filled both at home and a hospital. Next, you'll be subjected to urodynamics tests to see how well your bladder empties. Moreover, a nurse would carry a complete pre-op examination to give a verdict on your general health status.

Aftercare For Urethral Bulking

Having your urethra thickened with a bulking material, there’s no particular guideline as to which activities should be avoided. You can do physical exercise and resume your normal sexual routine. One thing that you might notice however is an increased urge to urinate, requiring more frequent visits to a toilet.

After the procedure, you should stick to the healthy diet to consolidate the results. Drink plentiful water (1.5-2 litres) every day. Avoid things that can irritate your bladder such as coffee, fizzy drinks, alcohol, acidic juices, tomatoes and chocolates etc. Moreover, eat plenty of fruits and vegetables and cereals to keep your bowel healthy, as constipation can seriously upset your bladder.

Also, you’ll likely be contacted by our support staff to give feedback on your experience, plus if the symptoms still persist. In case everything’s fine, you’ll be called up for a follow-up session 6 months after your urethral bulking procedure.

GENERAL Risks Associated with the Surgery

Risks with anaesthesia

As with any surgery, risks associated with local and more commonly general anaesthesia are retained with this procedure too. You are more prone to developing anaesthesia-related complications if you are a subject to certain medical conditions. We first analyse your condition and then put you on an anaesthesia to minimise the risk.

Urinary tract infections

Even if your surgical procedure has been performed well, you might still develop urinary tract infections after your incontinence treatment. As a matter of fact, this condition appears to be fairly common. If that's your case, you can always talk to our doctors and get any required assistance shortly.

Deep venous thrombosis

Anaesthesia is associated with a risk for clot development in deep veins (DVT). It is estimated at 4-5%. More so, sometimes this clot can migrate (embolise) to lungs, causing pulmonary complications. The risk however is low, accounting for only 1% of the cases. Our doctors inject you with anticoagulants such as heparin to minimise the risk.

SPECIFIC Risks Of URETHRAL BULKING Surgery

Failure of the surgery

Some women might not get the desired results even after a bulking procedure. They are usually advised to take multiple sessions to see if that brings them any relief in their symptoms. Besides, the bulking material might get infected and trigger an abscess. You’ll then need another operation to treat the infection and abscess. Even more, the bulking material can harden inside your urethra or bladder (although it’s very rare).

Difficulty in voiding

Because of a thick urethral wall, you might experience difficulty in voiding urine. Such women (around 5 percent) are put on a catheter to help them empty their bladder. The catheter is then removed within a few days after voiding becomes normal. However, if you face difficulty in voiding your urine even after ten days, you’ll be put on a small catheter and be taught how to empty your bladder using it, all by yourself.

Over-reactive bladder

An operation around the bladder has a potential risk of turning it over-reactive. If this happens, you’ll feel an increased urge to urinate, with more frequent visits to your toilet than usual. Apart from that, if you experience a consistent pain (lasting for more than 6 months) post your procedure (which usually results from the development of scar tissue at the injection site), you should contact us right away.
