Menopause is a condition when women no longer get periods and become unable to conceive naturally. It’s not very often for menstrual cycles to end abruptly and usually menopause is achieved after a spell of declining periods followed by complete cessation. 

In the UK, menopause on average is achieved by 52 years of age. However, some women can achieve it at 30 or 40. Menopause that sets in before the age of 45 is called a premature menopause. 

Menopause is a natural phenomenon and develops as you get older. The primary reason is a drop in the levels of estrogen hormones produced by the ovary with advancing age. Also, the ovary stops releasing eggs due to these hormonal imbalances.

The stop in monthly menstrual cycles may be abrupt. However, generally it sets in as a series of less frequent periods followed by a complete stoppage of menstrual bleeding.

Treatments such as surgical removal of ovaries (oophorectomy)

Certain treatments for breast cancer

As a complication of chemotherapy and radiotherapy

One very common problem after a menopause is progressive demineralization of bones or osteoporosis. The bones become soft and porous, increasing the chances of fracture. Bone resorption is very closely related to deficiency of estrogen, and osteoporosis clocks in when the levels of this hormone drop after a menopause.

However, sometimes it can also develop as a result of conditions such as surgical removal of ovaries (oophorectomy), chemotherapy or radiotherapy, underlying disorders such as Down syndrome or Addison’s disease etc.

What are the symptoms of menopause?

For some women, menopause does not cause significant symptoms. However, for others, it causes serious symptoms such as might sweats, hot flushes, vaginal dryness that can cause frequent infections, low mood and anxiety, a decreased sexual drive and problems with cognitions and focus. Whatever the symptoms may be, it is necessary to address them and get them resolved.

Does menopause cause osteoporosis?

Osteoporosis is associated with a deficiency of estrogen hormone. This causes excess bine resorption and leads to soft bones. Hence, osteoporosis is a very common condition observed after a menopause.

How long do the symptoms of Menopause Last?

The symptoms usually last for 4 years after the cessation of menstrual cycles. They can last even longer for some women.

How to treat symptoms of menopause?

If you are experiencing menopausal symptoms, you should see your GP. Most common treatment for menopause is a hormone replacement therapy (HRT). This therapy replaces the depleted hormones in the body and reverses the symptoms of menopause. Primary hormones that are given are estrogen and progesterone. These hormones are given both in combination or alone. This depends on the status of your uterus. Nevertheless, HRT is an effective treatment option for women who face severe symptoms of menopause.

Hormone Replacement Therapy (HRT) to have a Menopause clinic

As evident from its name, this therapy tends to make for the hormones that get depleted in the wake of a menopause. The goal is to restore hormonal equilibrium and revert any menopausal symptoms.

Hormone replacement therapy is also called estrogen replacement therapy (ERT) since estrogen is the primary focus. However, it also deals with restoring levels of other hormones, such as progesterone.

What’s quite interesting is that HRT also subsumes replacing testosterone levels in females.

Contrary to the popular belief, testosterone does play an important role in female’s good health and bodily functions. Although this role is not as significant as that in men. Hence, hormone replacement therapy is sometimes given in the form of a combination of hormones, which includes testosterone in its spectrum too.

Types of HormoNe Replacement Therapy (HRT)

In women who do have a womb, combined HRT, either continuous or sequential, is very important. If estrogen is given alone to such females, it constantly stimulates the womb (endometrium) to develop and thicken, which causes irregular shedding of the endometrial wall accompanied by bleeding. Moreover, such women are also prone to developing an endometrial cancer.

Oestrogen Only HRT

In females who have had a hysterectomy, we provide oestrogen only HRT. The rationale is that these women do not require progesterone to maintain the womb lining. Hence, they are given estrogen alone or sometimes in combination with progesterone.

Continuous Combined HRT

Women who have a uterus are given a combination of estrogen and progesterone. This way, the patient is given both estrogen and progesterone regularly. There are no withdrawal bleeds observed with this method.

Sequential Combined HRT

The patient is given estrogen for the first 14 days and is then given progesterone for the next 14 days to mimic a natural 28 days cycle. This method causes withdrawal bleeds that imitate natural regular menstrual bleeds.

Combined HRT on the other hand keeps overstimulation of the womb by estrogen in check. In sequential HRT, estrogen given for the first 14 days stimulates the womb. In the next 14 days, progesterone promotes regular shedding of the endometrium and prevents any endometrial cancer.

Similarly, continuous HRT makes sure that estrogen and progesterone keep each other in check with respect to their effects on the womb, all the time. This protects against any endometrial cancer and does not cause any withdrawal bleeds.

Formulations For HRT And Hormone Intrauterine Device

Hormone Replacement Therapy is given in the form of:

Tablets taken by mouth

Hormone patches applied to the skin

Hormone creams applied in the vagina

Intra-dermal hormone implants

Hormone releasing rings inserted into the vagina

These symptoms can manifest months or even years before the stop of menstrual bleeding. Moreover, they take a course of around 4 years after your last cycle or even more in some unfortunate women.
